First, what is a lumbar epidural steroid injection?
Patients may hear several names for the same general category of procedure: lumbar epidural steroid injection, LESI, ESI, epidural injection, steroid shot for sciatica, cortisone injection in the back, or a back injection for a pinched nerve. The precise approach can vary, but the basic idea is to place corticosteroid medication in the epidural space near an irritated spinal nerve to reduce inflammation.
Why does an epidural seem to help one person dramatically and do very little for another? The best current evidence says that epidural steroid injections probably reduce short-term pain and disability for some people with cervical or lumbar radiculopathy, but the average benefit does not predict one person’s result. The 2025 American Academy of Neurology systematic review also found limited evidence for durable long-term pain relief. That is why patient selection and realistic expectations matter.
In my work with lumbar-disc and sciatica patients, I see patterns that make me more or less optimistic about how much an anti-inflammatory injection may accomplish. Those patterns guide a conversation; they are not a test that guarantees success or failure. I do not perform epidural injections. When appropriate, I coordinate or refer patients to a qualified medical specialist for that decision and procedure.
A disc can irritate a nerve in two different ways
Imagine a nerve root passing through a doorway with limited space. A bulging or herniated disc can occupy part of that doorway and mechanically encroach on the nerve. At the same time, disc material and the surrounding tissue can participate in inflammatory and chemical irritation. Sciatica can therefore reflect a mixture of mechanical pressure and inflammatory sensitivity rather than one mechanism alone.
Research describes inflammatory activity as one contributor to disc-related radicular pain, although the relationship between individual inflammatory markers and symptoms remains complex. An epidural corticosteroid is intended primarily to address that inflammatory component. It does not push a disc back into place, immediately make a herniation disappear, enlarge the foramen, or remove structural compression. The North American Spine Society guideline supports epidural steroid injection as an option for short-term relief in some patients with lumbar disc herniation and radiculopathy; it does not describe the injection as structural repair.
That distinction is the foundation for a useful discussion. When inflammation is an important part of the pain, reducing it may create meaningful relief. When fixed mechanical compromise is dominant, an anti-inflammatory treatment may have less room to change the overall picture.
Clinical perspective
When I am more optimistic about an LESI
I am generally more optimistic when the patient has true radicular leg pain and the history, examination, and available imaging reasonably fit the same nerve-root pattern. I also pay attention to whether symptoms change meaningfully with position, movement, or load. Sitting may be worse than walking, a particular movement may send pain farther down the leg, or another position may bring symptoms back toward the buttock or low back. A comfortable position may still exist.
That variability does not prove an injection will work. It suggests that the presentation is mechanically modifiable and may not be explained by unchanging structural pressure alone. Centralization and directional preference are useful examination and prognostic findings in low-back presentations, but the updated systematic review did not establish them as treatment-effect predictors for an epidural injection.
Symptom duration may also matter, although it is not a firm rule. One retrospective study associated shorter symptom duration with a more favorable short-term response, while other studies have produced less consistent findings. A recent systematic review of response predictors concluded that the evidence varies in quality and risk of bias. MRI characteristics should therefore inform diagnosis and targeting, not be oversold as a crystal ball.

When I am less optimistic
I become more cautious when severe radicular pain is essentially relentless, there is almost no comfortable position, symptoms show little meaningful variability, weakness is significant or progressing, and examination plus imaging indicate substantial concordant mechanical compromise. Longstanding symptoms, minimal change after appropriate conservative care, or repeated well-targeted injections without meaningful sustained benefit also lower my expectations for another anti-inflammatory intervention.
The steroid can reduce inflammation around a compressed nerve, but it cannot remove the structure compressing it. Constant, relatively unchanging pain does not prove that an epidural will fail, but when it occurs alongside significant neurological findings and substantial structural compression, I tend to be more cautious about how much an anti-inflammatory injection alone can accomplish.
No single symptom or MRI phrase should make the decision by itself. The direction and pace of neurological change, functional loss, symptom duration, medical history, prior response, and the patient’s goals all belong in the discussion.

Why I like to make as much progress as possible before the injection
Assuming there is no neurological emergency and conservative care is appropriate, I prefer to make every reasonable gain we can before an injection. That may mean improving movement, reducing muscular guarding, identifying a directional preference, helping symptoms centralize, developing trunk and pelvic control, restoring activity tolerance, and teaching the patient which loads or positions aggravate and relieve the condition.
Therapy does not chemically make the steroid stronger. The strategy is simpler: address modifiable mechanical and functional contributors first. If persistent inflammatory nerve irritation still blocks progress, the corticosteroid then has a more focused job instead of being expected to solve every component of the condition.
I would rather make every reasonable gain we can before the injection so that, if we ultimately need the steroid, we are asking it to do one job instead of every job. This is a clinical strategy—not proof that pre-injection therapy increases steroid potency.
Therapies we may use before an epidural
Care is selected from the examination and the patient’s response. No one automatically receives every treatment below, and continuing an intervention that consistently worsens leg symptoms is not the goal.
- Directional movement and centralization: This is not simply backward bending. The clinician tests whether a direction changes symptoms. Pain retreating from the foot or calf toward the buttock or back is centralization; symptoms moving farther down the leg are peripheralization. Extension helps some people, while another direction may fit others. Learn more about sciatica evaluation.
- Selected lumbar traction: Evidence is mixed, and traction is not suitable for every disc presentation. It may be trialed for a selected patient while response is monitored. Improvement or centralization supports the choice; consistent peripheralization or worsening calls for reassessment. Traction does not put a disc back in.
- Stability and progressive exercise: The goal is not a vague claim that the core is weak. Physical therapy may address trunk coordination, endurance, hip strength, movement control, graded loading, walking, lifting, and work tolerance.
- Adjustments and manual care: Selected lumbar or pelvic restrictions and protective movement may coexist with a disc injury. Chiropractic care may help movement, pain, or tolerance for rehabilitation when appropriate. An adjustment does not push a herniated disc back into place.
- Dry needling: Dry needling may address a separate muscular or myofascial contributor involving lumbar, gluteal, or hip-rotator muscles. It does not decompress the spinal nerve or treat the disc itself.
- Class IV laser: Class IV laser therapy may be considered as an adjunct for symptom modulation while rehabilitation progresses. The available radiculopathy literature has important limitations and low-to-very-low certainty in a recent review; laser should not be described as disc repair, nerve regeneration, or a cure.

The injection can create a window
If an epidural substantially reduces leg pain, I do not tell the patient they are fixed. I say, “Great. We now have an opportunity.” Less nerve irritation may make it easier to walk, sleep, exercise, build strength, tolerate work, and participate in rehabilitation.
Injection and rehabilitation are not necessarily competing choices. An injection may create a window in which the patient can resume the active work that supports function. The plan should still be reassessed according to what the person can do and whether progress lasts.
What if the first epidural does not work?
A first injection that produces no lasting benefit does not automatically mean surgery. The treating medical team may reconsider whether the targeted level and diagnosis match the symptoms, whether even short-lived relief occurred, whether the clinical picture has changed, and whether inflammation still appears to be a meaningful contributor.
The question is not simply whether the procedure was a success or failure. A limited response can be information. It may shift attention toward diagnosis, rehabilitation strategy, another medical opinion, or the possibility that mechanical compression has become the more important issue. Decisions about another injection belong with the qualified clinician performing or managing that procedure.

What if two injections do not work?
There is no universal medical rule that two unsuccessful injections mean a patient needs surgery. I commonly see a surgical consultation become more reasonable when concordant imaging, persistent significant radicular symptoms, appropriate conservative treatment, one or more appropriately performed injections without meaningful sustained benefit, and continued functional limitation all point in the same direction.
When reasonable rehabilitation and an anti-inflammatory intervention have both failed to materially change a clearly compressive radicular condition, the question of structural decompression may deserve more attention. A consultation is an opportunity to understand options; it does not itself commit a patient to surgery.
Why the process sometimes looks like therapy → injection → surgery
Many patients encounter a progression from conservative treatment and rehabilitation, to an epidural when radicular symptoms persist, and then to surgical evaluation if substantial symptoms continue. This is not an absolute sequence, and an urgent neurological problem can change the timeline immediately.
Coverage rules also differ among Medicare, commercial plans, VA authorization, and individual policies. Many policies expect documentation of diagnosis, functional impact, and reasonable conservative management when medically appropriate. The current CMS epidural-injection coverage guidance is one example—not a universal rule for every insurer or patient. It would be inaccurate to say that insurance always requires two injections before surgery.
When we do not simply work through the steps
New bowel or bladder dysfunction, saddle sensory loss, rapidly progressive leg weakness, significant foot drop, severe bilateral neurological findings, or concerning systemic illness with spinal symptoms require prompt medical assessment. These are not situations in which a patient should simply finish a routine therapy sequence first.
Epidural corticosteroid injections also require a benefits-and-risks discussion with the clinician who would perform them. The FDA has warned of rare but serious neurological events and notes that corticosteroids are not FDA-approved for epidural administration. That does not mean every injection is inappropriate; it means informed consent and individualized medical judgment matter.

My preferred goal: make the injection optional if we can
When a patient comes to Relief Plus with a lumbar disc injury and sciatica, my first question is not how quickly we can get them an epidural. It is how much progress we can safely make without one.
If directional exercise, rehabilitation, selected traction, manual care, dry needling, laser, education, and progressive activity allow recovery without an injection, that is useful progress. If the patient improves but persistent inflammatory nerve irritation prevents further gains, an epidural may become an appropriate next tool. If a well-selected injection does not meaningfully change symptoms, that response can also inform the next decision. Our broader clinical approach is to choose the right level of care for the person in front of us.
The goal is not to avoid injections at all costs, and it is not to rush into them. The goal is to understand what is driving the patient’s symptoms and use the least invasive treatment that can reasonably move that person forward.

So when do I think an LESI tends to work best?
I consider the whole pattern rather than one symptom. These findings shape how optimistic or cautious I am when discussing referral for an epidural.
More optimistic
- True radicular leg symptoms
- Examination and imaging reasonably correlate
- Inflammation appears clinically important
- Movement, position, or load meaningfully changes symptoms
- Comfortable or relieving positions exist
- Symptoms can centralize
- Deficits are not rapidly progressing
- Conservative care has improved modifiable components
- Symptoms have not become indefinitely chronic
More cautious
- Relentlessly severe symptoms
- Virtually no positional change
- Significant or progressive weakness
- Major concordant mechanical compromise
- Longstanding symptoms
- Appropriate conservative care has produced minimal change
- Well-targeted epidural treatment has repeatedly produced little meaningful benefit
These are clinical considerations, not rules. No individual symptom pattern can guarantee whether an epidural steroid injection will or will not work.
